Over 30 Van Dyke Brown photographic prints created by Patrick Barrett will be displayed as part of an upcoming Brooklyn Public Library exhibit on alternative and historic photography techniques. An opening reception for Back To Basics will be held on Tuesday, March 28 at 6:00pm at the Central Library Branch, Grand Army Plaza. The exhibit runs through May 14, 2006 and also features the work of local artists Michel Bayard and Doug Schwab.

Patrick Barrett is a self-taught photo artist who concentrates on historic or alternative printing processes, often using low-tech and handmade cameras. As a Brooklyn resident, Barretts inspiration comes from the many buildings and structures that have become elements of living history, managing to retain their stamp on a neighborhood regardless of the shine or fast pace represented by their newcomer neighbors. His photojournaling of this collection of landmarks and architectural oddities led to an exploration of techniques that also held a compelling place in the present, despite origins in another era.    

Barretts use of the late 19th centuryVan Dyke process is an attempt to return these structures to their heyday. Many of the places pictured in this exhibit were being built or used when the technique was first being developed. Its process is deceptively simple, using basic running water and sunlight to develop. It is representative of a time when photography was coming into its own and of a time when a previously unknown pace of city living was taking hold of New York. Sturgeon Bay, Wisconsin Chiropractor and Naturopath Dr. J G Moellendorf, DC, ND, LCP, points out that the Baby Boomer Generation (born 1946 to 1964) has continually caused revolutionary adaptations in societys infrastructure as they age. There have been rapid expansions to service their needs, followed by rapid contractions. School systems and other infrastructure were overbuilt, only to be torn down or repurposed.

As the Baby Boomers came into the workforce, their taxes generated vast sums of money to support the Social Security and Medicare systems. These benefits radically expanded as there was no apparent end to this growth. Now as the Baby Boomers reach retirement, the US healthcare system must prepare for the impending explosion from increasing healthcare needs with fewer workers to pick up the bills.

In the January 2013 issue of the Journal of Manipulative and Physiological Therapeutics, researchers Smith, Davis, Stano, and Whedon published the results from analyzing data from a longitudinal Medical Expenditures Panel Survey of 71,838 respondents. They used this data to determine the future impact of an aging population on the trends in back pain and chronicity, along with estimates on the costs for treatment.

The researchers surveyed respondents for back pain at 4 to 6 month intervals across 2 year periods during 2000-2007. The respondents were divided into 5 separate groups: Elders (born before 1935), Preboomers (born 1935-1944), Boomers (born 1945-1964), Postboomers (born 1965-1975), and Younger Adults (born after 1975). They identified 8262 non-chronic and 3842 chronic cases of back pain. During the surveyed time period, overall cases of back pain increased 29%, while chronic back pain increased by 64%. When adjusted to 2010 dollars, costs increased by 129% from $ 15.6 billion in the 2000-2001 biennium, to $ 35.7 billion during 2006-2007.

The researchers also noted that back pain is second only to upper respiratory infections for why patients consult doctors. The prevalence of back pain, especially chronic back pain is rapidly escalating, principally because of an aging population. At retirement, 25% of Elders through Baby Boomers have chronic back pain, with 40% of this back pain becoming chronic. In recent years, the prevalence of back pain has been increasing by 8.3% annually, with 25% of sufferers accounting for 75% of costs.

The prevalence and chronicity of back pain threatens to strain an already overstretched US healthcare system. Smith et al. summarized researchers Deyo, Mirza, Turner, and Martin in their paper Overtreating Chronic Back Pain: Time to Back Off?: It is imperative to identify and dramatically improve the delivery of high-value care. The curative acute model often fails because many will never fully recover from back pain. A chronic care model demands better coordination of care which focuses on prevention, education, self-care, and functional improvement.

Publishing in the journal Pain, the research team led by Cynthia O Townsend used a multidisciplinary rehabilitation approach for chronic back pain averaging 9 years, which was able to achieve significant and sustained improvements in pain severity and function while withdrawing opioid usage. Townsends group notes that doctors treating back pain need to cooperate in developing multi-disciplinary guidelines that identify and encourage the delivery of the most efficient and cost-effective care.

The University of Pittsburgh Medical Center (UPMC) through its health plan has already taken steps in that direction. Realizing the high failure rate and extensive costs of surgery, the university has studied ways to increase success and minimize costs in treating chronic low back pain. On January 1, 2012, the health plan implemented a clinical initiative stating:

To be considered for surgery, patients with chronic low back pain must have:

    Tried and failed a 3-month course of conservative management, which includes physical therapy, chiropractic therapy, and medication.
    Completed UPMC Health Plans Low Back Pain Health Coaching Program.

Back pain is very common, affecting 85% of the U.S. population during their lifetime. Spine-related problems cost an estimated $ 85 billion yearly, along with uncounted hours in lost production. Yet, the current treatment for back pain is often uncoordinated, disorganized, and costly. Most sufferers will consult their medical doctor, receiving muscle relaxant, anti-inflammatory or pain medications, or if that fails, a referral for physical therapy or traction.

After repeated failures in treating their back pain, frustrated chronic back pain sufferers may turn to such alternatives as Chiropractors, Naturopaths, acupuncture, or massage. Each approach has its successesand its failures. Researchers are beginning to look into ways to increase the patients health and satisfaction, while hopefully reducing costs.

Dr. Niteesh Choudhry, MD, PhD of Harvard Medical School and Dr. Arnold Milstein, MD, MPH of Mercer Health and Benefits examined the costs and benefits of the various health care approaches to treating back pain. In order to evaluate overall treatment effectiveness and costs, researchers use the Quality Adjusted Life Year (QALY) to compare outcomes. A value of 1 would be symptom-free or perfect health, a 0 would be death, and 0.5 would mean moderate pain which limits some self-care abilities. The total costs of a treatment that yields an additional year of ideal health determines the cost per QALY. This incremental cost-effectiveness ratio can then be used to determine the comparative value of various treatments. Interventions with cost-effectiveness ratios below $ 50000 to $ 100000 per QALY are generally considered to be cost-effective.

Doctors Choudhry and Milstein reported in the Mercer Report that medical physician care for back pain cost $ 2355 (not including muscle relaxant, anti-inflammatory or pain medications) with an efficacy (QALY) of 0.618. Physiotherapy-led exercise had an efficacy of 0.635 at a cost of $ 3192. This results in an incremental cost effectiveness ratio of $ 49210 (increased cost divided by increased efficacy, numbers have been rounded). This is slightly below the $ 50000 to $ 100000 threshold for being acceptable as cost-effective. Chiropractic care had an efficacy of 0.659 at a cost of $ 2431, with an incremental cost effectiveness ratio of $ 1837. While Chiropractic was the most effective choice of treatment, it resulted in a slightly increased cost ($ 75) compared to medical care to achieve its effectiveness.

Because sufficiently detailed drug costs were lacking, drug expenditures were not included in this analysis. This would very likely have increased the medical costs compared to the Chiropractic costs, thereby actually causing a slight cost savings with Chiropractic care.

While it seems intuitive that adding physiotherapy-led exercise to Chiropractic care should increase its efficacy, it actually declined to 0.651 while raising overall cost by another $ 76 to $ 2507, resulting in an incremental cost effectiveness ratio of $ 4591.

Sturgeon Bay, Wisconsin Chiropractor Dr. J G Moellendorf, DC, ND, LCP notes, What makes the Mercer Report significant is that we have two highly respected medical researchers concluding that seeing a Chiropractor for back pain is not only the most effective treatment, but probably less costly than other forms of treatment!

The findings of researchers Nelson, Metz, and LaBrot were also noted in the Mercer Report. They observed that when Chiropractic care was utilized, there was reduced use of x-ray, MRI, CT scan, hospitalization, and surgery; resulting in substantial cost savings long term.

Jordan Hospital in Plymouth, Massachusetts studied the use of Chiropractic care in its low back pain program. Out of 402 low back patients, those treated by Chiropractors achieved clinically successful outcomes with an average of 5.2 visits, for only $ 302 per case. Pain and disability scores were decreased by approximately 70% in just a few weeks, while the satisfaction rate exceeded 95%.

The Mercer Report reported that nearly 50% of chronic spinal pain sufferers consult a Chiropractor for care. Chiropractic was found to be more effective than other modalities in treating back pain. When the cost of medications is added to medical physician expenses, Chiropractic care is probably also the least expensive treatment for low back pain. These results were further confirmed by the Jordan Hospital study.

On April 21st at 9 a.m. local charity Walk With Sally brought more than 20 children whos families have been affected by cancer to Hermosa Beach favorite restaurant Hot’s Kitchen for their Friendship in the Kitchen event. It was a day of empowerment, filled with educational fun courtesy of Chef Sean Chaney and surprise guest photographer Bo Bridges.

The morning started off with a tour of the kitchen led by Chef Sean Chaney where kids and mentors learned about the equipment in a professional kitchen and what its like to be a chef. Shortly after, surprise guest Bo Bridges gave participants a quick photography lesson and let them shoot some photos.

The main event was a cooking tutorial led by Chef Sean, where the kids were taught hands-on how to cook two easy and delicious dishes for their families Garden Fresh Salad and Shrimp Pasta. After a lunch of Hots specialty burgers and tacos, each child went home with a gift bag filled with the necessary ingredients and materials to prepare the meals at home, including kid-friendly cooking utensils from Kidding Around in the Kitchen.

Chef Sean Chaney was thrilled to be a part of the event saying, I had the best time today. It was so fulfilling to be able to teach these kids to make an easy and healthy meal. The way they got so excited about the little things we take for granted, like setting the table, or helping clean up, was humbling to see.

Pamela Penson, the CEO and President of Walk With Sally commented, Id say about 80% of the children here today have never learned how to cook or really set foot in a kitchen, so I believe todays event was truly life-changing for them. These kids have been searching for a way to give back to their families who have been so affected by the loss of a loved one to cancer, so by enabling them to go home tonight and cook a meal for their parents is incredible.

The Samaritan Inn, the largest homeless program in Collin County, is hosting a benefit concert at the Cotton Mill at 610 Elm St. in McKinney, Texas on Friday, April 27, 2012. The concert lasts from 7:00 pm until midnight. A similar benefit concert last year was a huge success, and event planners expect another great turn out at this years Rock the Inn 2.

Tickets to the event are $ 50 per person and include dinner & heavy hors doeuvres from Chef Pete Catering, cocktails, a cigar bar, and complimentary valet parking. Guests can also participate in a silent auction and a dance contest. The Emerald City Band, a 13-person-plus-crew rock and roll special event band, will be playing live during the event.

All the proceeds from “Rock the Inn 2″ go to The Samaritan Inn. This organization provides services to men, women, and families in the community who are experiencing homelessness.

Rock the Inn is truly a party with a purpose; a fantastic evening for our guests all to benefit families experiencing homelessness and living at The Samaritan Inn, said Executive Director Lynn Sipiora. Have fun, support the cause; win, win!

In addition, Juan Carlos Entertainment, a professional photography company, will be present at the event. He will give guests the opportunity to pose for a professional-quality photo that can be purchased after the event.

Even if you cannot attend this event, you are encouraged to still donate to The Samaritan Inns cause.

The Samaritan Inn was established in 1984 after community leaders joined together to address the issue of homelessness in Collin County. Twenty five years later, The Samaritan Inn is still the largest homeless program in a county of over 762,000 residents.
